The four-field message builder runs in your browser. This static page does not submit your selections to Cellforce One Clinic Tokyo or to a form endpoint.

What the message builder does

• It turns four selected answers into a short text summary.
• It lets you copy that summary to your device clipboard.
• It opens the clinic's official LINE route only when you choose the LINE button.

What it does not do

• It does not upload medical records, images, test results, contact details, or the generated summary.
• It does not confirm an appointment, make a diagnosis, or decide medical suitability.
• It does not place the four selected answers in the page URL.

Attribution data

If this page is opened with campaign parameters such as utm_source or utm_campaign, the page may retain those limited attribution values for the current browser session and include them in local analytics events when analytics is configured. The generated patient message does not include those values.

When you open LINE

LINE is an external service with its own privacy terms. Review the generated summary before you paste and send it. Do not send full records, identification documents, or detailed test values unless clinic staff explain an appropriate route.
